Looks like people are keying in on the wrong part of the anatomy. I guess that is because ear rhymes with engineer. That isn't it. It is in the same part of the body as the two given examples: foot and toe.
Another thing that may be throwing people off is their idea of what engineer's do. Don't think of today's electrical or mechanical engineers who design and build our smart phones and mobile devices. Think of civil engineers who build buildings like the original World Trade Center in the 60s. That's when I first heard this riddle.
You could also insert architect here instead of engineer for the same answer, but that is probably giving the clue away.
